Higher cancer mortality found among pilots and flight attendants in major U.S. study

Pilots and flight attendants show higher mortality from several cancers associated with ionizing radiation exposure, according to a large U.S. study published in JAMA Internal Medicine on August 17, 2026. Researchers examined mortality data across hundreds of occupations to determine whether aviation crews face a different cancer burden from the wider working population. The findings showed that flight attendants and pilots ranked particularly high for deaths from cancers classified by the researchers as potentially related to radiation exposure. The study is notable because aircrew members receive the highest average annual effective dose of ionizing radiation among occupational groups in the United States, primarily because of cosmic radiation encountered at commercial flight altitudes.

One possible explanation is the cumulative exposure to cosmic ionizing radiation during thousands of hours spent at cruising altitude. Earth’s atmosphere provides substantial protection at ground level, but that shielding becomes weaker as altitude increases. For an occasional airline passenger, exposure during individual flights remains relatively small. Pilots and cabin crew, however, can spend many years working at high altitude, meaning their cumulative occupational exposure can be considerably greater.

The findings should not be interpreted as proof that cosmic radiation alone causes the higher cancer mortality observed among aircrew. The researchers themselves note that whether occupational radiation exposure directly translates into increased cancer mortality has remained uncertain. Other characteristics of aviation work, including night shifts, irregular schedules and repeated disruption of circadian rhythms, may also contribute to long-term health risks.

The study therefore provides an important signal rather than proof of a simple cause-and-effect relationship. Its findings strengthen the case for continued research and more detailed monitoring of occupational exposure among people who spend much of their working lives at altitude. The broader question is whether modern aviation should develop more systematic long-term health monitoring for pilots and cabin crew as scientists gain a better understanding of the combined effects of radiation, disrupted sleep and years of working in an unusual high-altitude environment.
